New patients are always welcome at The Rycote Practice.

Is there a Practice Area?

We look after patients who live within our practice boundary. This includes residents of Thame, Emmington, Ickford, Kingsey, Long Crendon, Milton Common, Shabbington, Sydenham, Tetsworth, Tiddington, Towersey, Waterstock and Worminghall.

It is important that if you change your address you let us know. If you leave the area you will need to register with another doctor.

How do I Register?

In order to register as a patient you simply need to fill in a form and bring it in along with 2 forms of identification. The form can be collected from the desk or can be downloaded by clicking on the link below. Each family member, including children, requires a completed form. We no longer require NHS cards.

Although someone else can bring the completed application in to the practice, to complete registration each person being registered must be seen by our reception team, and they must also show 2 forms of identification. Identification is also needed for children. This is an NHS requirement.

Help filling in the form is available if required - just ask at the desk.

What happens once I've filled in the form?

Once you have handed in your form to register you will be able to use our services straight away. We recommend that you arrange to have a 20 minute appointment for a 'New Patient Health Check' as soon as possible. This can be with a member of our nursing team. If you are on repeat medication we recommend making your new patient health check appointment with a GP. You do not need to have had your new patient health check before you come to see a nurse or GP about a problem.

It can take 8 weeks to get your notes from your previous GP - so please let us know about any important illnesses, allergies and medications. This information can be entered on your registration form.

Will I be registered with a particular GP?

You will be registered with the practice rather than an individual GP, and you may see any of the doctors. You can express a preference of doctor and we will endeavour to fulfil this whenever possible.

We encourage patients with on-going health problems to choose and stay with a 'usual doctor' which allows us to get to know you and your health problems better. However, you can always choose to see another doctor should you wish to do so.

Can I be seen as a Temporary Patient?

If you have friends or relatives visiting who need a doctor and who are normally resident in the UK, we would be pleased to accept them as a temporary patient. Please tell the receptionist they are a temporary resident so they can arrange for them to complete the appropriate form.

For visitors not normally resident in the UK, they may be accepted for immediate NHS treatment if there is a medical emergency. For non-urgent cases they may be seen as a private patient and charges will apply.

Anyone who is resident in the area for more than 6 months is entitled to be registered with a GP in the area.
